It would seem more likely that it's Anthonys song/poem to John, warning him of the dangers of heroin, mentioning the women in his life as he often does and showing his affection for a band member/friend, as is often done in RHCP songs. It s also showing John that theres life after heroin with inspirational sayings ("I'll make it to the moon if I have to crawl"). Great display of what happens after a fractured brotherhood is put together again for a second time – egos put aside, near old age, maturity at an all time high.
