In her role as both a forensic psychiatrist and psychotherapist, Professor Adshead has tried to understand the psychological mechanisms that give rise to violence and life threatening behaviour toward others. She has worked as a member of a therapeutic team whose role is to rehabilitate and offer secure psychiatric care to some of the most vilified and socially rejected members of society. A compassionate work, the author succeeds in the difficult task of making the reader feel some empathy with the array of offending characters.The power of group therapy to assist in the rehabilitation of some is uplifting and an important message to populist politicians who like to be seen to be hard on criminals by incarcerating ever larger numbers of the mentally ill, depressed, abused and disadvantaged.

Deeply moving . . . the most overwhelming feeling I had on finishing this book was of hope . . . Compassionate and fascinating.' Dr A doesn’t indulge in too much therapy-speak (thank you for that Dr A!) but she is inclined to throw out general statements such as I gazed at him, this man who so wanted to talk and who felt things so deeply. I thought about how removed he was from the image I’d once had of the ruthless and unfeeling serial killer. Most people who suffer mental health are often harmless to society and are at greater risk of harm from others than the rest of society. The author is so compassionate and full of wisdom in how she helps these people.

It was certainly eye-opening in a number of ways, and sometimes heart-breaking, but unfortunately as a whole it didn't work all that well for me. Note that this is clearly an unpopular opinion, as my fellow readers all loved it. Some cases I found fascinating, others much less so. I disliked the sometimes flowery language, and things like the literary quotes the doctor would think of when she stepped into a room for a consultation that just bugged me and took me out of the experience. Again, this is clearly me being overly sensitive. I also felt that it was somewhat one-sided, monotonous, lots of X or Y grew up in bad circumstances and was exposed to this and that horror and here we are today, with X or Y having committed this or that crime. Sometimes, as one reads, the struggle to understand feels at odds with a reflex moral outrage. You might want to retort: “This story doesn’t excuse it!” (Whatever “it” might be.) But The Devil You Know is not a book of excuses. It persuades us that it is only through understanding why horrific crimes happen that mental health services and the judicial system can have any chance of being improved. This revelatory book encourages us to see that it is our responsibility to consider the worst of humanity – and of ourselves. And while we are at it, it urges us to hang on to Adshead’s most powerful imperative: “the duty of hope”. The patients Adshead are some unbelievably evil people, and some of what I learned of these people was hard to read. It's difficult to take into account that if a person is concluded to be mentally ill at the time of a horrific murder, somehow there is more sympathy for that person in a way. It shouldn't actually matter, the person killed another human being, and their families have to live with that forever.When we plant a seed no matter how special it might be if it's not put in the right soil, receives the right amount of sunshine and water, the best nutrients, it isn’t going to grow to be a beautiful orchid or whatever flower the seed would become. Instead, it will grow up broken rather than beautiful. The author believes that a lot of early trauma and family disruption can lead to great harm as an adult.
