In patients with a known hypersensitivity to ibuprofen, paracetamol or any other excipients in the product. Ibuprofen is a non-selective inhibitor of cyclooxygenase, an enzyme invovled in prostaglandin synthesis via the arachidonic acid pathway. Its pharmacological effects are believed to be due to inhibition cylooxygenase-2 (COX-2) which decreases the synthesis of prostaglandins involved in mediating inflammation, pain, fever and swelling. Antipyretic effects may be due to action on the hypothalamus, resulting in an increased peripheral blood flow, vasodilation, and subsequent heat dissipation. Inhibition of COX-1 is thought to cause some of the side effects of ibuprofen including GI ulceration. Ibuprofen is administered as a racemic mixture. The R-enantiomer undergoes extensive interconversion to the S-enantiomerin vivo. The S-enantiomer is believed to be the more pharmacologically active enantiomer. In patients with severe hepatic failure, severe renal failure or severe heart failure (NYHA Class IV) (see Section 4.4). To minimise side effects, it is recommended that patients take Nuromol Dual Action Pain Relief with food. The product provides more effective pain relief than paracetamol 1000 mg in short-term treatment (p<0.01) and long term treatment (p<0.01).Paracetamol is also thought to relieve pain by reducing the production of prostaglandins, this time in the brain and spinal cord. These prostaglandins sensitise nerve endings, so that when that area is stimulated it causes pain (presumably to prevent us from causing further harm to the area). As paracetamol reduces the production of these nerve sensitising prostaglandins it is thought it may increase our pain threshold, so that although the cause of the pain remains, we can feel it less. Paracetamol doesn't reduce inflammation. Please note, your healthcare professional is best placed to provide you with advice regarding your health.
