Izumi singles Mei out for blame at the inn, demanding she apologize for all the deaths she's caused. In the film, their relationship is considerably worse, as Kirika constantly discouraged Mei from socializing with her classmates, telling her she didn't need friends, and she shouldn't explain about her eye as people would judge her for it. In July, after realizing that the countermeasures to the Class 3-3 calamity are not working, he kills his bedridden mother by smothering her with a pillow the night before stabbing himself in the neck in front of the class. It all comes to a head in episode 10 when Izumi demands that Mei apologizes for her perceived part of the calamity, despite Kouichi's, Teshigawara's, and Mochizuki's protests.

Mei also says that her doll's eye sees the "color of death", her proof that Kōichi is not the "extra". Though she wasn't one of the students who initially sought out Mei following Takako's broadcast, Izumi became actively hostile following Takako's death, as she thought Mei had deliberately killed her.Tomohiko accuses Naoya of fucking Yuuya, which causes him to panic and flee out a window, only to become entangled in a loose wire and electrocuted. Mei and Naoya, who were trying to escape the inn, immediately find them, but are unable to help her as Izumi and Tomohiko confront them next.

However, despite telling Kouichi to stay away from her, as time goes on, she becomes accustomed to his presence and doesn't seem to mind having him around.Izumi, upon seeing the knife in Mei's hand and Takako's corpse, instantly believes that not only is Mei the extra, but also thinks she stabbed and hung Takako. As an honors student who was also good at sports, the charming girl was popular with her classmates.

Distant, icy, but ultimately concerned for the fate of her classmates, Izumi introduces the countermeasures to protect the class from death. However, as they part ways, Misaki suddenly collapses, later being admitted into hospital and dying from leukemia. Kouichi says something similar to this to Izumi in Episode 10, when she tried to place the blame on Mei.

Kōichi's father asks him over the phone how it feels to be back in Yomiyama after being gone for a year, but Kōichi says it is his first time here. Despite being the catalyst for Takako and the others attacking Mei in the anime and the main reason for her loneliness in the manga and novel, Misaki does not appear in the live action film, nor is she mentioned. Her hobby is drawing, and she is a member of the school's art club, though she states that she doesn't enjoy creating art so much as she enjoys looking at it. Devastated by the loss, the students and teacher of class 3-3 continued to behave as though Misaki was still alive, leading to a strange presence shown within the graduation photo.
