Players select which three cards out of the six they want in hand and on the table. Play for Winner The first player is the player who receives a three face-up. If there is not a 3 face-up, the first player to declare they have a three in hand is the first player. If there are no 3s dealt, repeat with four (and so on). Twisted Shithead is an elaborate variation in which nearly all the cards have special effects. It was created by Galbraith/Lewis/Constance/Mabely (1993); the description is by Paul JaYmes (1997).

If they cannot match or beat the previously played card with their face up cards, they must take the pile as usual. They cannot play any face up cards again until their hand is empty. The first person to play a 10 or complete a four of a kind, clearing away the pile, is known as the "flush" person, and has to clear away the pile each time this happens in future.

When an eight is played, the direction of play is reversed (so its function is similar to that of the joker in the main description). In this variation an eight can be played on any card. Eights are transparent - when playing on an eight, you must beat the first card under it that is not an eight. Deal each player three cards face down (set these side by side for each player). Players DO NOT look at these cards. This game can accommodate 6 players if jokers are used. Jokers may be played at any time and they reverse the direction of the play. Jokers are not wildcards and can not be played with other cards. Dealing Variation The game of Head Poo challenges players to throw plush poo’s at the other player head who will be wearing a Velcro hat, the aim of the game being to catch as many poo’s as possible.

You can put a five on top of everything, it is the reverse card, meaning that in the next turn the opponent must put a card equal or lower than five. If someone completes a set of four cards of the same rank on top of the discard pile (either by playing all four cards at once or by equalling the previous play), the whole pile is removed from play, and the same player who completed the four of a kind takes another turn, playing any card or set of equal cards to start a new discard pile.
