Hook begins teaching Peter's children about why parents hate their children; though he is unsuccessful with Maggie, the neglected Jack finds some truth in Hook's words. His hand is cut off not by Peter, but accidentally severed when he slams the lid of a trunk in a fit of rage. The film's story line is based on the books written by Sir James Matthew Barrie in 1904 or 1905 and is the sequel to the first book. Other stories featuring the captain include those centering the Disney Villains franchise (such as the tongue-in-cheek book The Villain Files) and various books serving a Smee serves as an adviser to Hook, assisting in his murderous aspirations to kill Rumplestiltskin and his aspirations to obtain information and raise the newly arrived Baelfire.

The character was only completely pinpointed long into the animation stage, as it proved difficult to create a menacing villain whose character would not be diminished by the scenes with the Crocodile.

He also talks Hook into trying to persuade Peter's children, Jack and Maggie, to remain in Neverland permanently by using their father's repeated broken promises and prolonged absences as examples. This happens when Peter and Wendy appear to make Goofy's dream for some adventure come true and play a game of "Pretend to Be Pirates" with Donald Duck, who pretends to be the captain until the real Hook appears and challenges Peter to a duel. As Captain Hook thanks Tinker Bell for her help, he locks her in a lantern, then he, along with Smee and the pirates, goes to Hangman's Tree to capture Wendy, Michael, John and The Lost Boys.

Soon as Peter prepares to attack, Hook then engages in a sword fight with him in battle just as Wendy, Michael, John, the Lost Children, Tinker Bell, and Tiger Lily join the battle and fight off against the pirates. Wendy tries to convince Hook to be friends with Peter, which Hook denies, telling him that his time for joy is lost and everything he could have been has been reduced to his state when he grew up wrong. However, Riku reports to him from Maleficent that Wendy is not a Princess of heart at all, irritating Hook (he hints that kidnapping Wendy was a very difficult task). After Gold travels to Manhattan to locate Baelfire, Hook poisons him, but is captured by Neal Cassidy (Baelfire)'s fiancé Tamara, to be used in her and Greg Mendell's plot to destroy magic in Storybrooke.

Near the end of the film, a man bearing a striking resemblance to Smee is seen sweeping garbage in Kensington Gardens, to Peter's surprise, but whether or not he is the real Smee is left unclear. Only upon receiving Wendy's kiss, and five weeks' worth of sleep, does the real James Hook again reveal himself. This last promise is kept when he pulls a single hair from Peter's head, declaring "the rest of him is mine".
